Central Neighborhood Health Foundation is currently hiring several Community Outreach Workers at multiple CNHF locations based in Los Angeles, Riverside, and San Bernardino Counties!
JOB DESCRIPTION SUMMARY:
Under indirect supervision, works closely with medical providers, primary care teams, and social services agencies to provide short-term care coordination and connection to resources and support to program HHP members to improve their health and general well-being through education and provision of coordination of care and services. Works in both clinical and community-based settings, including HHP member’s homes.
Paraprofessional or peer advocate, administrative support to a Care Coordinator, high school diploma or GED; at least 3 years of experience directly related to the duties and responsibilities specified.
EDUCATION AND/OR EXPERIENCE:
High school diploma or GED; at least 3 years of experience directly related to the duties and responsibilities specified.
